Agents and Brokers The Health Reinsurance Association will pay Agents or Brokers a one time fee of $50.00 to assist Connecticut residents in the application process. The Agent or Broker must be licensed in the State of Connecticut The last section on the back of the application form titled "Please
provide us with the following information, if a licensed insurance agent
assisted in the completion of this application" must be completed
and sent to the Health Reinsurance Association at the time application
for the program is made.
