TAA


2009 Enrollment Criteria

All applicants must be a Connecticut resident under the age of 65.

Eligible Dependents Are:

· Your Spouse
· Your unmarried children (biological or adoptive) under nineteen years of age (twenty-three if a full time student attending an accredited institution of higher learning) who depends on you for support.
· Any other unmarried child (biological or adoptive) under nineteen years of age (twenty-three if a full time student attending an accredited institution of higher learning) who depends on you for support and lives with you in a regular parent child relationship.


There are four possible situations for enrollment.

1. Individual Plans

Individual Plans. The plan options are the Special Health Care plan and the UNITEDHealthCare PPO plan. If you have not had group or qualifying individual insurance coverage in the last 150 days, you may enroll in these plans. These plans will not cover pre-existing conditions for one year. Please refer to the brochure for an explanation of a medical pre-existing condition.

2. Conversion Plans

Conversion Plans. The plan options are the Special Health Care Conversion plan, the UNITEDHealthCare PPO Conversion plan, and the Health Net HMO Conversion plan. These plans will cover pre-existing conditions immediately.

You may enroll in the Conversion plans if you meet the following requirements:

  1. You have had employer group coverage, or qualifying coverage, for at least 12 months.
  2. Your application and premium are received in our office within 120 days of the end of your prior health insurance plan due to voluntary loss of coverage or 150 days of the end of your coverage due to involuntary loss of coverage. The HRA plan will begin coverage the 1st of the month following receipt of a completed application.

Qualifying Coveragemeans the following:

Any group health insurance plan, or group insurance arrangement, or self insured plan covering a group or
Medicare or Medicaid or
An individual health insurance plan that provides benefits which are actuarially equivalent to or exceeding the benefits provided under a small employer health care plan, as defined in section 38a-564, whether issued in this state or any other state.

3. Portability Plans

Portability Plans. The plan options are Special Health Care Portability plan, the UNITEDHealthCare PPO Portability plan, and the Health Net HMO Portability plan. These plans will cover pre-existing conditions immediately.

You may enroll in these plans if you meet the following requirements:

  1. You have had at least 18 months of prior creditable coverage.
  2. You have exhausted your other group insurance options, such as COBRA.
  3. Your application and premium are received in our office within 120 days of the end of your prior insurance plan due to voluntary loss of coverage or 150 days of the end of your prior health insurance due to involuntary loss of coverage. The HRA plan will begin coverage the 1st of the month following receipt of a completed application.

4. TAA Plans

TAA Individual Plans

Individual Plans. The plan options are the Special Health Care plan and the UNITED HealthCare PPO plan. These plans will not cover pre-existing conditions for one year. Please refer to the brochure for an explanation of a medical pre-existing condition.

You may enroll in these plans if you meet the following requirements:

  1. Individuals who have not had group or qualifying individual coverage in the last 150 days.
  2. You qualify for the federal Health Coverage Tax Credit.

TAA Portability Plans

Portability Plans. The plan options are the Special Health Care Portability plan, the UNITED HealthCare PPO Portability plan and the Health Net HMO Portability plan. These plans will cover pre-existing conditions immediately.

You may enroll in these plans if you meet the following requirements:

  1. Your application and premium are received in our office within 120 days of the end of your prior insurance plan due to voluntary loss of coverage or 150 days of the end of your prior insurance coverage due to involuntary loss of coverage.
  2. You have had at least 3 months of prior creditable coverage.
  3. You qualify for the federal Health Coverage Tax Credit.
